4-Hydroxy propofol is a short-acting intravenous anesthetic agent. It has been shown to inhibit the activity of cytochrome P450 enzymes and protein synthesis in rat liver microsomes. 4-Hydroxy propofol also inhibits recombinant cytochrome P450 enzymes, and this inhibition is reversible. The metabolite of 4-hydroxy propofol, propofol O-glucuronide, may be responsible for its hepatotoxicity. Women are more sensitive than men to the effects of 4-hydroxy propofol because they have higher levels of estrogen and lower levels of testosterone. This drug also inhibits the production of uridine in humans, which can cause toxicity. There are genetic polymorphisms that affect the metabolism of 4-hydroxy propofol among humans.
