(2S,4R)-4-Hydroxy-L-glutamic acid is a metabolite of L-glutamic acid. It is synthesized in the body by the enzyme glutamate carboxylase and is found in blood, urine, and cerebrospinal fluid. (2S,4R)-4-Hydroxy-L-glutamic acid has been shown to be an important biomarker for renal function. The glomerular filtration rate (GFR) is the volume of fluid filtered from the blood by the kidneys into the urine per unit time. A metabolic pathway that can be used as a marker for GFR is glyoxylate to form oxalic acid which can then form calcium oxalate crystals in the kidney and urinary tract. This process can lead to kidney stones and renal failure. The GFR can also be measured using creatinine clearance or cystatin C levels.
