3-Hydroxy-4-trifluoromethylbenzoic acid is an irreversible inhibitor of the enzyme 3-hydroxy-3-methylglutaryl coenzyme A reductase, which is involved in biosynthesis of cholesterol. This drug has been evaluated in a number of clinical trials for use in acute ischemic stroke. 3HTB was shown to be effective in reducing the size of brain infarcts and improving neurological function at doses that do not cause significant side effects. It also has antiplatelet activity, which may be due to its ability to inhibit platelet aggregation and prolong clotting times. 3HTB inhibits microglia activation by inhibiting production of inflammatory cytokines such as tumor necrosis factor alpha (TNFα) and interleukin 1β (IL1β). It also inhibits LPS induced inflammatory response by blocking NFkB translocation into the nucleus.
