Methyl-malonyl-coenzyme A (MMA) is a coenzyme that participates in the metabolism of propionate. It is produced by the mitochondrial enzyme methylmalonyl-CoA mutase. This enzyme converts methylmalonyl-CoA, which is an intermediate in the citric acid cycle, to MMA. The population of patients with high levels of MMA are at risk for vitamin B12 deficiency and may be more susceptible to antibiotics, such as erythromycin. 13C NMR spectroscopy has been used to detect increased levels of MMA and propionylcarnitine in patients with chronic renal failure. These studies have also shown that there is a correlation between high levels of MMA and gender (female).
