(R)-Mephenytoin (MEPH) is a stereoselective prodrug of phenytoin that is used in the treatment of epilepsy. The conversion to phenytoin from MEPH occurs in the liver by cytochrome P450 enzymes. The clinical pharmacology and therapeutic efficacy of MEPH have been studied in healthy Chinese, as well as urine samples obtained from patients with epilepsy. Studies on rats have shown that this drug has a high affinity for p-450 isozymes, which are highly polymorphic in humans. These studies also show that human liver microsomes metabolize MEPH to phenytoin more efficiently than rat liver microsomes.
