2-Methyl hippuric acid is a metabolite of benzoic acid and phenylacetic acid. It is produced by the oxidation of benzoic acid or the metabolism of phenylacetic acid, and excreted in urine. 2-Methyl hippuric acid can be used to detect infectious diseases, such as HIV and hepatitis B. This test is based on hydrogen bonding interactions between 2-methylhippuric acid and picolinic acid. The 2-methylhippuric acid concentration in human urine increases with cancer diagnosis, due to increased uptake from cancer cells. Urinary concentrations of 2-methylhippuric acid are also increased in patients with metabolic disorders such as diabetes mellitus and renal disease.
2-Methyl hippuric acid levels are influenced by the matrix effect (i.e., urinary pH). For example, an acidic environment increases the solubility of this compound, which leads to higher concentrations in urine samples
