S-2-Pyridin-4-ylethylcysteine (SPEC) is a biochemical marker of pulmonary fibrosis. It can be measured in blood, urine and tissues. SPEC is a noninvasive method for the diagnosis of idiopathic pulmonary fibrosis (IPF). The measurement of SPEC in prospective studies has been shown to have high sensitivity and specificity for diagnosing IPF. SPEC can also be used as an indicator of disease progression and response to treatment. The measurement of SPEC is a potential biomarker for other lung diseases that are characterized by increased extracellular matrix deposition, such as emphysema or bronchitis.
