(R)-Propafenone is a drug that is used to treat atrial and ventricular arrhythmias. It has been shown to have a potent inhibitory effect on the cardiac sodium channel, which can be stereoselective in nature. This drug also inhibits the release of calcium from the sarcoplasmic reticulum, which may lead to decreased blood pressure. (R)-Propafenone has been shown to decrease water vapor loss in humans, which may be due to its ability to inhibit the breakdown of ATP by inhibiting oxidative phosphorylation. It also has electrophysiological effects on human ventricular myocardium in vitro. (R)-Propafenone binds with high affinity to human liver and enantiomerically pure form is needed for clinical use.
