Tramadol is a centrally acting analgesic and opioid. It is used for the treatment of moderate to severe pain and as an adjuvant to anesthesia. Tramadol is rapidly absorbed from the gastrointestinal tract, with only about 20% of the dose being excreted unchanged in urine. Tramadol undergoes extensive first-pass metabolism, which results in low systemic bioavailability. The metabolic pathways are mediated by cytochrome P450 isoforms and involve N-dealkylation, O-demethylation, N-oxidation, and S-oxidation. These processes lead to multiple metabolites such as tramadol-d6 (TMD6) that are active or have been shown to have analgesic effects in animal models. TMD6 has an elimination half-life of 3 hours, which is long enough for it to be detected in wastewater samples using liquid chromatography coupled with tandem mass spectrometry (LC/MS/MS). TMD
