S-Sibutramine is a drug that was originally used in the treatment of obesity and has been found to be effective for the treatment of congestive heart failure, pulmonary hypertension, and bowel disease. It is a centrally acting agent with 5-HT2 receptor antagonist properties. It blocks the reuptake of serotonin from the cleft between the nerve terminal and the target cell by competitive inhibition, which leads to an increase in serotonin concentrations in the cleft. This mechanism is thought to be responsible for its therapeutic effects on both bowel disease and pulmonary hypertension. S-Sibutramine also has pharmacological effects on drug metabolism as it can inhibit CYP3A4 activity in vitro, leading to increased concentrations of drugs such as warfarin or phenytoin that are metabolized by this enzyme.
