Pivmecillinam is an antibiotic that belongs to the group of penicillin-binding proteins. It has shown a high degree of activity against gram-positive bacteria and some gram-negative bacteria. Pivmecillinam binds to bacterial penicillin-binding proteins, which inhibits their ability to form crosslinks with cell wall synthesis enzymes, leading to cell lysis. This drug is also used in combination with amoxicillin or ampicillin as an oral prodrug for the treatment of urinary tract infections caused by susceptible strains of Escherichia coli and Proteus mirabilis. Pivmecillinam has been shown to have a low potential for ecological effects due to its rapid metabolism and excretion in the urine.
