(R)-4-Cyano-3-hydrobutyric acid ethyl ester is a chemical compound that is used as an antidiabetic agent. It is also known as eflornithine, and has been approved by the FDA for the treatment of African sleeping sickness. (R)-4-Cyano-3-hydrobutyric acid ethyl ester has shown to be effective in reducing insulin resistance and improving glucose tolerance in mice. The mechanism of action is not fully understood, but it may be due to its ability to inhibit the enzyme ornithine decarboxylase, which plays a role in the synthesis of polyamines. This drug has also been shown to reduce fat deposits in mice with diet-induced obesity. (R)-4-Cyano-3-hydrobutyric acid ethyl ester has also been shown to improve muscle function and increase muscle mass in vitro studies.
