Big endothelin-1 (Big ET-1) is a peptide that regulates the vascular tone. It is a monomer with a molecular weight of 39 kDa. Big ET-1 is present in lung epithelial cells and alveolar macrophages. Big ET-1 has been shown to be an endogenous endopeptidase inhibitor, cleaving big endothelin 40 into big endothelin 37 and big endothelin 38. This enzyme also functions as an extracellular neutral endopeptidase, cleaving bradykinin, angiotensin II, and substance P. Big ET-1 may play a role in the pathogenesis of bronchiolitis obliterans syndrome (BOS), which is characterized by inflammation and fibrosis of the small airways that results in severe respiratory impairment.
