(R)-Mirtazapine is the (S) enantiomer of mirtazapine, which is an antidepressant. It is a potent antagonist at presynaptic alpha-2 adrenergic receptors and postsynaptic 5HT2A receptors. The effective dose of (R)-mirtazapine ranges from 15 to 45 mg per day. Plasma concentrations are undetectable within 24 hours after taking a single dose of (R)-mirtazapine. Clinical pharmacology studies have shown that it increases serotonin levels in the brain by preventing its reuptake into the presynaptic neuron and by blocking serotonin degradation. The drug has been shown to be effective in various genetic types, including genotype 1 and 2 erythrocytes.
