4-Cyano-4-[(ethylsulfanyl)carbonothioyl]sulfanylpentanoic acid (CETP) is a cholesteryl ester transfer protein inhibitor that prevents the transfer of cholesteryl esters from high-density lipoprotein to low-density lipoprotein. It also inhibits the synthesis of cholesterol and phospholipids in macrophages, resulting in decreased atherosclerotic lesion formation. CETP is used as an adjunct to diet and exercise for the treatment of high levels of cholesterol and triglycerides, specifically in patients with type IIa or type IIb hyperlipidemia. It has been shown to increase HDL cholesterol levels by decreasing the rate at which cholesteryl esters are transferred from HDL to LDL. This process inhibits atherosclerosis by reducing the amount of LDL circulating in the bloodstream.
