Olmesartan methyl ester is a prodrug of olmesartan medoxomil, which is a selective angiotensin II receptor antagonist. Olmesartan methyl ester was developed as a research chemical for drug development and to explore the role of intramolecular proton transfer in the mechanism for its activity. It has been shown to have pharmacological effects on the cardiovascular system by inhibiting angiotensin II-mediated vasoconstriction and vasodilation. The absorption and elimination rates are dependent upon the pH of the stomach, with higher pH values leading to increased absorption and decreased elimination. The rate of metabolism is dependent upon the hepatic enzyme CYP2C9, which converts olmesartan methyl ester into its active form olmesartan medoxomil. This conversion also depends on the pH of the stomach, with higher pH values leading to faster conversion and lower concentrations in plasma.
