CAS:Cis-4-hydroxy-lomustine is a carbamylated prodrug that is converted to the active form, lomustine, by hydrolysis. It has high lipophilicity and is readily taken up by tissues. Cis-4-hydroxy-lomustine inhibits the growth of cancer cells by binding to DNA and blocking RNA synthesis. The drug also inhibits the activity of pyridinium crosslinks in proteins and nitroimidazole drugs in microorganisms. Cis-4-hydroxy-lomustine does not undergo extensive metabolism in the body, but undergoes rapid conversion to the active form, which is excreted mainly as unchanged drug in urine. Cis-4-hydroxy-lomustine has been shown to be effective against colorectal cancer cells with wild type p53 tumor suppressor gene and against small cell lung cancer cells with mutant p53 tumor suppressor
