It can be synthesized by a regioselective synthesis from 3-hydroxypropanoic acid and butyric acid. Tert-butyl 4-hydroxybutyrate can also be synthesized by tethering hydroxylamines to prodrug form of tert-butanol. Tert-Butyl 4-hydoxybutyrate is a prodrug form of the drug, Terbumost, which is an esterase inhibitor used to treat ulcerative colitis. This drug has been shown to selectively inhibit the activity of esterases in the gastrointestinal tract and thereby reduce inflammation by inhibiting prostaglandin synthesis.
