Trimethoprim is a bactericidal antibiotic that inhibits bacterial growth by interfering with the synthesis of folic acid. It binds to the 30S ribosomal subunit and prevents the formation of an antibiotic-inhibitor complex with the enzyme, which is required for cell wall biosynthesis, inhibiting protein synthesis and cell division. Trimethoprim has been shown to have cardiac effects, including QT prolongation and torsades de pointes ventricular tachycardia, in some patients. The drug also has been shown to inhibit the growth of bacteria that produce benzalkonium chloride, such as Pseudomonas aeruginosa. Trimethoprim may be used in combination with polymyxin B or colistin to treat infections caused by Gram-negative bacteria that are resistant to other antibiotics.
