Cotinine is a metabolite of nicotine that is found in the blood. It is used as a biomarker for exposure to tobacco products and has been shown to be an independent risk factor for cardiovascular disease. Cotinine can be detected in the blood, plasma, saliva, urine and breast milk with detection times of 24 hours to one week. Cotinine can also be measured in hair samples with detection times of 80 days or more. The measurement of cotinine levels in biological samples is complicated by the presence of other metabolites from nicotine metabolism and its high affinity for proteins such as albumin. It is important to account for these confounding factors when interpreting results from cotinine measurements. The most common method used to measure cotinine levels in biological samples is liquid chromatography-mass spectrometry (LC-MS/MS).
