1',4-Dihydroxy midazolam is a metabolite of diazepam. It is formed by the oxidation of the 1'-hydroxyl group in the benzene ring to an alcohol and a subsequent hydrolysis of the alcohol to a carboxylic acid group. This metabolite has been shown to have low affinity for binding with cytochrome P450 enzymes in vitro and in vivo, which may be due to its high polarity. The 1',4-dihydroxy midazolam is rapidly eliminated from plasma and has a short half-life. The clearance rate for 1',4-dihydroxy midazolam is higher than that for diazepam, suggesting that this metabolite may be responsible for some of the therapeutic effects observed with diazepam treatment.
