8-Epitacrolimus is a macrolide immunosuppressant drug that has been used to prevent rejection of transplanted organs. It inhibits the activity of T cells by binding to cytosolic protein p21Ras and blocking the activation of the cell cycle. The compound is also a potent inhibitor of protein synthesis, leading to decreased production of inflammatory cytokines such as IL-2 and TNF-α. 8-Epitacrolimus has been shown to be effective in preventing acute graft versus host disease (GVHD) in patients with hematopoietic stem cell transplants, but it is not active against bacterial infections because it does not bind well to bacterial ribosomes.
