Indinavir is a protease inhibitor used to treat HIV infection. It binds to the active site of the protease and prevents it from cleaving peptide bonds, which are necessary for viral replication. Indinavir also inhibits p-glycoprotein (P-gp) which is an efflux pump protein that pumps drugs out of cells. By inhibiting P-gp, indinavir prevents it from removing itself from the cell, thereby increasing its amount in the body and prolonging its half-life. Indinavir is metabolized by hepatic cytochrome P450 enzymes into three metabolites: indinavir sulfate, indinavir glucuronide and indinavir sulfamate. These metabolites have been shown to inhibit human immunodeficiency virus type 1 (HIV-1) replication in vitro. The compound has also been shown to be effective against opportunistic fungal infections such as Candida albicans and Aspergillus fumigatus.
