Mitotane is a drug that inhibits the function of p-glycoprotein (P-gp), which is an important protein in the transport of drugs and other substances across cellular membranes. Mitotane has been shown to inhibit P-gp function and increase the uptake of mitoxantrone, doxorubicin, and daunorubicin by adrenocortical cells. Mitotane may be used to treat adrenocortical carcinoma by inhibiting tumor growth and preventing metastasis. This drug also has a cytostatic effect on adrenocortical cells, slowing or stopping cell division. It is thought that this inhibition may be caused by mitotane's ability to alter mitochondrial functions. The molecular pathogenesis of mitotane induced cytostasis is not well understood at this time, but it may involve a matrix effect on mitochondria.
