Pilocarpine is a cholinergic drug that can be used to treat eye disorders, such as glaucoma. It has been shown to have anti-inflammatory properties by inhibiting the release of pro-inflammatory cytokines. Pilocarpine also has water permeability and radiation sensitivities that are similar to those of acetylcholine. The drug is not recommended for use in patients with autoimmune diseases or chronic dry eyes, as it may worsen these conditions. Pilocarpine can cause drug interactions when taken with other drugs that are metabolized by the cytochrome P450 enzyme system (e.g., cimetidine). The drug's effects on the central nervous system are due to its ability to activate muscarinic receptors in the caudate putamen and hippocampal formation and inhibit acetylcholine release from nerve terminals in the brainstem and spinal cord.
