Sitagliptin phosphate monohydrate is a DPP-IV inhibitor that is used to treat type 2 diabetes mellitus. Sitagliptin decreases postprandial glucose levels and improves glycemic control. It binds to the enzyme DPP-IV, which breaks down peptides in the small intestine, inhibiting the breakdown of incretin hormones such as glucagon-like peptide 1 and glucose transporter type 2 (GLUT2). This leads to increased levels of GLP-1. Sitagliptin also has been shown to have an anti-diabetic effect by increasing insulin release from pancreatic beta cells.
