Tenofovir is a prodrug that is converted to tenofovir diphosphate (TFV-DP) by cellular enzymes. TFV-DP binds with high affinity and specificity to the viral DNA polymerase, thereby inhibiting the enzyme's activity. Tenofovir is used in the treatment of HIV/AIDS and other infectious diseases. It has been shown to be safe and effective for long-term use in patients with HIV/AIDS. A prodrug of tenofovir called tenofovir alafenamide hemifumarate may be an alternative for patients who cannot tolerate or have developed resistance to other antiretroviral drugs. The drug has shown no toxic effects at doses up to 1,200 mg per day when administered orally for 28 days. It is also possible that TFV-DP binds with higher affinity and specificity than TFV to nuclear DNA, which may account for its antiviral effects on HIV replication.
