Troleandomycin is a macrolide antibiotic with antibacterial activity. Troleandomycin is a stable drug that can be administered orally and has a long half-life, which makes it suitable for chronic use. Troleandomycin inhibits cytochrome P450 3A4 (CYP3A4) and P-glycoprotein (Pgp), which are important enzymes in the metabolism of other drugs. It has been shown to have an active inhibitory effect on human liver microsomes and to inhibit the growth of mouse monoclonal antibody cells, but not human lymphocytes. The mechanism of action of troleandomycin is not fully understood; however, it may be due to inhibition of DNA polymerase and RNA synthesis by binding to the bacterial ribosome at the site where tRNA binds.
