±-Methyllevulinic acid is a treatment for patients with type 2 diabetes that is used to lower blood sugar levels. It has been shown to work by inhibiting the activity of phosphoenolpyruvate carboxykinase, which is an enzyme that catalyzes the conversion of oxaloacetate to phosphoenolpyruvate and inorganic phosphate. This inhibition leads to a decrease in the conversion of glucose into glycogen and an increase in the production of glucose-6-phosphate, leading to elevated levels of glucose-6-phosphate. ±-Methyllevulinic acid has also been shown to inhibit zirconium diphosphate synthase, which catalyzes the formation of zirconium diphosphates from phosphoenolpyruvate and pyrophosphate, thereby preventing ATP production.
