Diphenhydramine is an antihistamine drug that is used to treat allergic reactions and bowel disease. Its mechanism of action is not fully understood but it may involve the inhibition of histamine release from mast cells, preventing the release of other chemical mediators such as prostaglandins and leukotrienes. Diphenhydramine also interacts with many other medications, including drugs that are metabolized by cytochrome P450 enzymes. One study found that diphenhydramine inhibits the activity of guanine nucleotide-binding proteins and this may be responsible for its sedative effects. Diphenhydramine citrate (DPHC) is a prodrug that undergoes hydrolysis by esterases to form diphenhydramine hydrochloride (DPH). The analytical method for DPHC involves colloidal gold agglutination in the presence of an anhydrous sodium sulfate matrix.
