It has also been shown to affect the activity of many different enzymes, including cytochrome P450s and monoamine oxidases. Rac N,o-Didesmethyl tramadol is not found in the urine of humans following oral administration but is found in wastewater treatment plant effluents. This compound has been shown to be toxic to human cells in vitro and in vivo.

Rac N,O-Didesmethyl venlafaxine has been shown to have a stereoselective effect on ventricular dysfunction in mice with heart failure. It also has a high affinity for opioid receptors and can cause drug interactions with other drugs that are opiate receptor agonists such as codeine or tramadol.
