Ramelteon is a melatonin receptor agonist that has been shown to be effective in the treatment of insomnia. Ramelteon binds to the melatonin receptors in the suprachiasmatic nucleus, thereby suppressing the release of histamine and other stimulatory neurotransmitters. Ramelteon is metabolized by hepatic cytochrome P450 enzymes and has been shown to have drug interactions with drugs such as erythromycin, ketoconazole, itraconazole, fluconazole, clarithromycin, and grapefruit juice. The drug can also cause side effects such as headache, dizziness, nausea, fatigue, somnolence, dry mouth or throat. This drug is not recommended for people with chronic obstructive pulmonary disease (COPD) because it may worsen lung function.
