Elisartan is a drug used to treat high blood pressure. It is an angiotensin receptor antagonist, which blocks the binding of angiotensin II to its receptors in blood vessels and tissues. Elisartan is a prodrug that is metabolized by butyric acid amidohydrolase to elsartan, its active form. Elsartan inhibits the activity of angiotensin converting enzyme (ACE), which breaks down the peptide hormone angiotensin I into the potent vasoconstrictor angiotensin II. The inhibition of ACE leads to increased levels of angiotensin II, which causes blood vessels to constrict and increases blood pressure. Elisartan also has been shown to inhibit neprilysin, a metalloprotease that degrades enkephalins and thus may be responsible for some of elisartans effects on pain perception.
